Registration for all 2026-27 sports is now open!!

Sign-up online: FinalForms

1).  FinalForms:  FinalForms allows you to complete and sign athletic participation forms for your student(s) for fall, winter, and spring sports - today! This means FinalForms saves data from season-to-season and year-to-year, meaning you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

Click on the Parent Playbook to get started in FinalForms. If you require any support during the process in FinalForms, click Get Help.

2). Make sure you have a current physical that is good through the entire sports season. You will be asked to upload your athlete's physical to FinalForms or you can turn in a hard copy to the athletic office.

3). Medical Orders & Health Plans—If your student has required emergency medications at school for a severe medical condition such as asthma, anaphylaxis, seizures, and or diabetes in previous school years, WA state laws states we must have a medical order, medication, and health plan in place prior to their participation in any in-person activities, including athletics. With the start of a new athletics season, your student may need additional items completed before they are eligible to participate. Please contact your student’s school nurse with any questions.
